SUBJECT: Missing Record Audit
STATUS: Internal Notice
A routine audit of recovered materials has identified multiple references to records that cannot currently be located within the Archive database.
The affected references appear in witness statements, evidence logs, preservation reports, and prior administrative correspondence.
Personnel are advised that the existence of a reference does not confirm the existence of a corresponding record.
At this time, the following procedures are in effect:
- Do not remove references to unavailable records.
- Do not alter file numbering to compensate for gaps.
- Do not create replacement records.
- Do not assume missing records were destroyed.
All references to unavailable files are to remain attached to the original documentation.
Several reviewed records contain citations to file numbers that do not appear within the current Archive index.
Additional records reference memoranda that cannot be located.
One audit report references a previous audit report that is not present in any known repository.
No evidence of deletion has been identified.
No authorized removal orders have been located.
No record of transfer has been recovered.
Personnel are reminded that missing documentation should be treated as an unresolved condition rather than an administrative error.
The Records Audit Division will continue its review.
A supplementary report will be issued if additional discrepancies are discovered.
